Authorization for the Sale of Hametz

This form must be completed and submitted before noon on Monday, April 22, 2024.
In accordance with Jewish law, I/we authorize Rabbi Bolton to sell all hametz in my/our possession at the address above plus at the locations listed below.

This box must be checked in order to execute this contract.

By entering my printed name I acknowledge that electronic signatures have the same legal effect as ink signatures.

It is traditional to make a contribution when selling hametz to help provide Matzah for those who cannot afford it. You are not obligated to contribute, and any amount is welcome.
